How Do I Eligible for CDPAP?
- Live in New York State
- Be eligible for New York State Medicaid
- Be currently receiving or approved to receive long-term home care service by the Department of Social Services

CDPAP Application Process
- Determining Eligibility: You must have Medicaid and require home health care in order to be eligible for CDPAP.
- Select a Fiscal Facilitator (FI): An FI is a business that assists in managing the program's administrative elements, such as B. payroll and taxes. A list of FIs is made available to you by Medicaid.
- Find a Caregiver: Friends and family members are acceptable candidates to serve as your caregiver. An investigation on their past and training are required for your caregiver.
- Complete Application: Your FI will give you the forms you need to finish the application procedure, including a prescription from a doctor.
- Plan of Care: Your exact needs will be listed in a plan of care your doctor will develop, along with the responsibilities of your caregiver.
- Approval: You can start getting services through CDPAP as soon as your application has been reviewed and accepted.
